Navigator Team: Identifying Wrap-around Services
Our pilot program includes a small Navigator team. On our first visit we gave the participants an intake form, very similar to the DOC Reentry planing documents, but simplified.
The team began investigating how best to meet the needs of each participant based on their intake information.
In week four, the team was able to meet the second participant to be released, and is now in the process of assisting the participant under the direction of DOC Community Navigator Kara Norsworthy.
Collaboration
A key part of the Navigator component in our pilot program is working closely with the DOC Navigator at the facility where it’s being piloted. Our team had the opportunity to meet with Janet Whisman, the Navigator at WCC.
One goal is to create as comprehensive a repository of services—including state, federal, local, business, and nonprofit resources—and identify potential and existing obstacles, as well as redundancies and overlaps, that impact successful reentry support and wraparound services.

Our team continues to make connections, build relationships and gather information in determining how best the pilot program could provide practical assistance from the initial release of a participant in our program.
